2. Health Checkup Items for Foreign Teachers: Height, weight, vision, blood pressure, body temperature, physical examination, syphilis (RPR), chest X-ray, measles, and rubella antibody.

I. Category 1 Children: Children aged 6 and below registered in Taipei City, with one of the parents being registered and residing in Taipei City for at least 2 years. For in-person applications, fax or mail the original household certificate of the child and either the father or mother.

II. Category 2 Children:

A. Original household certificate of the child and either the father or mother.

B. Other supporting documents:

1. Low-income households: Children aged 6 and below registered in Taipei City holding a low-income household certificate.

2. Special cases: Children aged 6 and below registered in Taipei City holding proof documents issued by Taipei City Department of Social Welfare.

3. Rare disease patients: Children aged 12 and below registered in Taipei City with a diagnosis certificate.

4. Major illness patients: Children aged 12 and below registered in Taipei City holding a National Health Insurance Major Illness Notification or a Major Illness Card.

III. Category 3 Children: 

Children aged 6 and below, born as the third child of the family and are registered in Taipei City. (To apply for medical subsidy for Category 3 children, please go to any district household registration office in Taipei City.)

A. If the applicant is a person with no legal capacity or limited legal capacity, the legal representative should sign or stamp on the application form and attach an identification document (verified by Citizen Digital Certificate for online application); if there are multiple legal representatives or guardians, only one is required to sign or stamp, a letter of attorney or letter of consent from the other legal representatives or guardians should be attached.

B. An original copy or a photocopy of the household certificate.

IV. Application submitted by a proxy:

A. Identification of the proxy (verified by Citizen Digital Certificate for online application).

B. An original copy or a photocopy of the identification document of the applicant.

C. The letter of attorney. 

V. Vaccination appointment and record (yellow card) or other relevant vaccination-related documents.

1. Pap smear screening is available once a year for women who are over 30 years old and sexually active.

2. Oral cancer screening is offered once every two years for individuals over 30 years old, specifically limited to those who have chewed betel nut or smoked.

3. Mammography is available once every two years for women over 45 years old.

4. Fecal occult blood test is available once every two years for individuals over 50 years old.

5. An annual elderly health check-up is available for individuals over 65 years old.

6. One subsidized expanded screening for hepatitis B and C.

7. A subsidized health check-up once every three years for individuals aged 40 to under 65.

2. New immigrants must bring their resident certificate and their spouse's identification (with the spouse being a registered Taipei City resident) to a contracted medical institution. They must also fill out a consent form and undergo the checkup.

II. Medical services covered by the National Health Insurance:

A. Treatment for diseases, injuries, childbirth, and accidents, outpatient or inpatient medical services. Patient that seek medical attention in accordance with relevant medical regulations and bear a portion of the medical expenses.

B. Child preventive health care, adult preventive health care, cervical smear tests for women, prenatal checkups for pregnant women, and other preventive health care services. For preventive health care services, part of the medical costs are exempted.

2. If the new immigrant has not yet obtained a resident certificate, they should first contact the National Immigration Agency to apply for one before applying for the prenatal checkup subsidy.

A. If you are unemployed but legally eligible for coverage as a dependent through a working spouse or direct blood relative, you should contact the entity managing your spouse's or relative's insurance to arrange for your dependent coverage.

B. If you are unemployed and lack a spouse or direct blood relatives for insurance dependency, visit the local township, city, or district office where your household registered to apply for National Health Insurance.

For information about the special area for household waste drug inspection and collection stations, please go to the "Environmental Cleaning/Household Drug Waste Inspection and Collection Station Area (Health Bureau)" at the bottom of the screen on the Environmental Protection Bureau's website (https://www.dep.gov.taipei/). As for syringes that do not come into direct contact with blood or body fluids, they are considered general waste. Please seal them in special garbage bags and hand them over to our cleaning team for removal.

（1）For those who are unemployed, if you are qualified as an dependent of the spouse or direct blood relatives to be enrolled, you should register at the insured unit of your spouse or lineal descendant to enroll as an dependent.

（2）If you are unemployed and cannot be enrolled through employed spouse or direct blood relatives, you can register at local administrative offices (villages, city or district) where your households are registered.

(1) Down Syndrome Screening:

Pregnant new immigrant women can apply for one of the following screening subsidies:

9-13 weeks pregnant: NT$2,200 subsidy

14-20 weeks pregnant: NT$1,000 subsidy

(2) Genetic Diagnosis Tests and Counseling for Prenatal Screening:

High-risk pregnant women can receive a NT$5,000 subsidy for amniocentesis testing.

(3) Prenatal Examination Fees for New Immigrants Not Yet Covered by the National Health Insurance:
