1. Application qualifications and required documents (for verification and photocopying)

(1) Application by myself: original of ID card or other documents sufficient to prove identity, original of other documents sufficient to prove identity: such as household register, health insurance card with photo, etc. * Foreigners must have the original of passport or residence permit

(2) Agent application: original of patient’s ID card, original of agent’s ID card, original of patient’s authorization letter * Foreigners must have original passport or residence permit

(3) Application for legal representative (from January 1, 2020, in accordance with Article 12 of the Civil Code of China, an adult is over 18 years old): Original of the patient’s ID card (minors may provide the original of the household register or a copy of the household registration Original), original ID card of the legal representative, documents proving the relationship with the patient (household register, transcript of household registration or original court ruling, etc.) * If this item is applied by an agent, the above information and the authorization letter and the agent must be prepared Original ID card

  (4) Application by a person with the right of inheritance (his spouse, direct relatives, collateral relatives or legal representative): original ID card of the person with the right of inheritance, documents proving the relationship with the patient (original ID card, household register or household registration transcript, etc.), patient Original of proof of household removal (transcript of household removal or death certificate) * If an agent applies for this item on your behalf, the above information and the consent letter must be prepared, as well as the original of the agent’s ID card * Foreigners must have the original passport or residence permit

2. Fees for various application items: (1) General diagnosis certificate: Chinese: 100 yuan per copy, photocopy per copy 15 yuan, English: 200 yuan per copy, photocopy per copy 15 yuan

  (2) Birth certificate: Chinese: first issuance; first three copies 20 yuan, each copy from the fourth copy 15 yuan, photocopy 15 yuan each, English: 200 yuan each, photocopy 15 yuan each

  (3) Death certificate: Chinese: first issuance; first three copies are NT$20, each copy from the fourth is NT$15, photocopies are NT$15 each, English: NT$200 each, photocopies are NT$15 each

  (3) Injury diagnosis certificate: 300 yuan each, photocopy 15 yuan each

  (4) Medical certificate: NT$20 each

  (5) Proof of medical expenses: Chinese: NT$50 per copy, NT$15 per copy from the second copy onwards, English: NT$200 per copy, photocopies NT$15 per copy

  (7) Photocopying of medical records (examination/inspection report): 1. Photocopying of medical records without consultation: basic administrative fee of 180 yuan (including 10 pages), and an additional 4 yuan per page from the 11th page. For example: Mr. A wants to print a medical record with 12 pages. The total charge is 180 yuan + 2 pages * 4 yuan = 188 yuan. 2. Medical consultation and photocopying of medical records: 4 yuan per page (two pages for double-sided photocopying).

  (8) Photocopies of discharge medical record abstracts: 50 yuan per copy, and 4 yuan per page for the second photocopy of the medical record (two-sided photocopies total 2 pages).

  (9) Medical record abstract: Chinese: 400 yuan per copy, English: 600 yuan per copy

  (10) Image disc duplication (X-ray, CT, MRI): NT$200 per item (if the capacity of each item exceeds one disc, additional fees will be charged based on the number of additional discs)
