I. Pregnant women who are residents of Taipei City:
1. Application form.
2. Applicant's ID card.
3. Maternity Health Handbook
- The page with the applicant's name.
- The page with the expected due date.
- The page with the latest prenatal checkup record, which must have the signatures of the prenatal care institution and the doctor.
OR
- A diagnostic certificate issued within the last month, specifying the gestational weeks and expected due date.
II. Foreign pregnant women whose spouse is a registered resident of Taipei City:
1. Application form.
2. Applicant's resident certificate.
3. Maternity Health Handbook, including:
- The page with the applicant's name.
- The page with the expected due date.
- The page with the latest prenatal checkup record, which must have the signatures of the prenatal care institution and the doctor.
OR
- A diagnostic certificate issued within the last month, specifying the gestational weeks and expected due date.
4. The ID card of the spouse.
5. The application form must include the spouse's name, National ID number, and registered address.
